Diabetes and Health Education Center
If you struggle with problems such as diabetes, asthma, depression or other chronic conditions, Mission Hospital’s Diabetes and Health Education Center is here for you. Our outpatient facility enables you to successfully manage your own care. By providing knowledge and support, our nurses and dieticians team up with a behaviorist or certified social worker to find the most effective ways to help you.

Mission Hospital provides comprehensive support programs and treatment plans for people with chronic medical conditions such as diabetes, asthma, depression, high blood pressure or cholesterol and COPD. Explore how our effective services can help you successfully manage your chronic condition.

The Health Education Center also coordinates programs with employers. These employer-sponsored disease management programs mobilize teams that work with the Health Education Center to find solutions for your life. These teams include you and your physician, a community pharmacist or health care professional and the sponsoring employer (who provides financial support).
Participants may be self-referred, referred by a physician or an employer.
