Our nurses, physical, speech, and occupational therapists, medical social workers, home health aides, and other professionals work with your doctor to provide the care you need, in the place you prefer -  at home.

To qualify for home health care:

  • Patients must meet definition of Homebound
  • Patients must need skilled care that is medically reasonable and necessary
  • Services must be ordered by a physician
  • Patient must have face-to-face encounter with ordering physician

CarePartners Palliative Care

CarePartnes Home Care & Hospice palliative care is often about the relief of your pain and other symptoms. From helping you breathe more easily, to guiding stressful situations, to tapping our own clinical skills, to you from returning to the hospital, there's very little palliative care doesn't assist.

Palliative Care is a consultation service, which means your regular physician still guides and oversees all aspects of your care. From helping to alleviate the fear you feel long after a long hospital stay to facing new challenges at home, we offer much-needed sympathy, compassion and care to you and your family.

CarePartners Hospice

Hospice is a special way of caring for individuals who are in the final stages of their lives due to a life-limiting disease such as end-stage heart, stroke, or a terminal illness such as cancer. Hospice services help patients who no longer wish to seek aggressive treatment such as chemotherapy and radiation or a medical cure, and desire management of their symptoms at home.

Choosing Hospice care early can be a comfort to you and your family. Our Hospice team consists of our medical director, nurses, social workers, chaplains, hospice aides, bereavement coordinator, and volunteers who are here to bring support to you and your family every step of the way. The focus of our care is compassionate comfort which includes pain management, medical care, personal care, emotional, and spiritual support. We coordinate all services with your physician, including inpatient and respite care, if needed, so continuity of care is maintained, 24-hours a day, 7-days a week.

To qualify for Hospice care, your physician must be able to certify that you have a terminal illness with six months or less life expectancy.
